Outreach Request Form
 
Students, staff and faculty are welcome to request an educational workshop/program or outreach (tabling or presence at an event) from CAPS. 

To make an outreach request, please complete the following form.

For any educational workshop or program, please review the list of offerings we have crafted, to see if one of these can meet your needs. With your active collaboration, we can also offer tailored outreach for your group, class or student organization.

Availability to accommodate requests varies over the course of the semester. Please allow at least 3-4 weeks advance notice. We will do our best to accommodate your needs, however, note that we may be unable to meet your request due to other commitments and obligations. 

(If your need is related to a crisis or otherwise urgent situation, please do not complete this form and instead either go to walk-in hours CAPS Monday-Friday 9AM-4PM or after-hours and weekends please call 855- 256-7160).
